How to make a nomination

Nomination statement (max 500 words)

The nominator should provide a clear statement addressing:

  • Nature of the innovation or research: What has been developed or discovered?
  • Purpose and originality: How is it new or significantly different from existing practice or knowledge?
  • Impact: Evidence of improved patient outcomes, service delivery, or professional practice
  • Dissemination and scalability: Has the work been shared (e.g. publications, conferences, CPD)? Can it be applied elsewhere?
Supporting evidence (required attachments) 
  1. One testimonial from a collaborator, colleague, or stakeholder (max 500 words)
    • Highlighting the nominee’s role and the impact of the innovation or research
  2. Optional evidence (max 2 items):
    • eg, Abstracts or papers, service evaluation reports, implementation frameworks, case studies, funding award letters, media coverage
